The Individual Coverage Health Reimbursement Arrangement (ICHRA) is a powerful and flexible alternative to traditional group health insurance. Established by federal rule in 2020, ICHRA allows employers of any size to fund tax-free HRA dollars that employees can use to purchase their own individual health insurance — either on the Illinois ACA marketplace or outside it. For many Illinois small businesses, ICHRA eliminates the complexity of managing a group plan while still providing meaningful health benefits to employees.

What Is an ICHRA?

An ICHRA is an employer-funded account — not an insurance policy. Each month, the employer contributes a set dollar amount to each employee’s HRA. The employee uses those funds to pay premiums for an individual health insurance plan of their choosing, as well as eligible out-of-pocket medical expenses. Employees who do not spend their entire HRA allowance lose the balance at year end (or carry it over, if the employer allows).

ICHRA replaced the older Qualified Small Employer HRA (QSEHRA) for some use cases and introduced several key improvements, including no contribution limits and the ability for large employers to use it.

Key ICHRA Features

  • No minimum or maximum contribution limits: Employers set any contribution amount — from $50/month to $2,000/month per employee.
  • 100% employer tax deduction: Contributions are fully deductible as a business expense.
  • Tax-free for employees: Employees receive HRA reimbursements free of federal income tax and payroll tax (if the plan is structured correctly).
  • Any ACA-qualified individual plan qualifies: Employees can choose any plan they want — marketplace plans, off-exchange plans, COBRA, or in limited situations, Medicare.
  • Works for ALEs: Unlike QSEHRA, ICHRA can satisfy the ACA employer mandate for employers with 50+ FTEs if the HRA offer is affordable under IRS guidelines.

Illinois ACA Marketplace Carriers Employees Can Choose

When employees use their ICHRA funds to shop the Illinois ACA marketplace (GetCoveredIllinois.gov), they can choose from the following carriers (availability varies by county):

  • Blue Cross Blue Shield of Illinois (BCBSIL) — broadest statewide network
  • Cigna Connect — available in select Illinois markets
  • Molina Healthcare — competitively priced, urban markets
  • Ambetter (Centene) — value-tier marketplace plans

Employee Classes: Design Flexibility

A powerful ICHRA feature is the ability to set different contribution amounts for different classes of employees, as defined by IRS regulations. Permissible classes include:

  • Full-time employees vs. part-time employees
  • Salaried vs. hourly employees
  • Seasonal employees
  • Employees in different geographic areas (e.g., Chicago vs. downstate)
  • Employees covered by a collective bargaining agreement
  • New hires in a waiting period

This means a Chicago tech company could offer full-time downtown employees $600/month while offering part-time remote employees $300/month — all within the same ICHRA framework. Rules prohibit discrimination within a class, but contribution levels can vary across classes.

When ICHRA Works Best for Illinois Employers

ICHRA is particularly well-suited for Illinois employers who: have a geographically dispersed workforce across different counties or states; struggle with high group plan renewal increases; have a workforce with diverse healthcare needs where a one-size-fits-all group plan is inefficient; or want to offer health benefits without the administrative burden of managing a group policy. It is also a strong fit for startups and small businesses that want to offer immediate health benefits without the 2-employee minimum that some group carriers require.
